Goal Cards
Tools to Strengthen Families and Communities
 

Aiding individuals and families as they assess the issues that impact their lives and set specific goals for addressing these needs helps motivate them toward change. Family workers will find Goal Cards beneficial for supporting families as they take a meaningful look at the factors impacting their lives, prioritize their needs, and set goals. The tool offers an alternative way to think about the issues impacting their lives and build the confidence and capacity they’ll need to accomplish their goals.

Assessment and goal-setting are integral parts of most programs working with individuals or families. For many, identifying goals is difficult. This user-friendly tool provides a structured process to identify specific goals and select a few to work on. The individual, family or worker should feel free to add goals not included in the cards.
