Partnership Self-Assessment Survey Tools to Strengthen Families and Communities
Developing cross-agency collaboration can be a difficult task because each organization is often not clear on its role and how the relationship will benefit each partner. Leadership and staff of community-based partnerships can use this assessment tool to track the progress of new partnerships and facilitate a dialogue. Results from the self-assessment will provide insight into the quality of partners' efforts and identify areas in need of improvement.

Partnership Self-Assessment Survey
The following assessment tool is designed to facilitate a dialogue among partners. It is generally helpful for individuals to complete the assessment by themselves first and then share their responses with the larger group. This reduces group pressure to agree up front. After filling out the self-assessment, group members should utilize the results to identify priority areas for improving their efforts together. Full text» [PDF 1MB]
