Some programs do not have strong connections
with other programs. The programs may be isolated by distance,
philosophy, and even language. Jargon, or acronyms and other
program-specific terminology, makes communication difficult.
Planning transitions can be very frustrating for both staff and
families without these connections. Transition planning meetings may
be ineffective because of poor communication among programs. Parents
may have difficulty obtaining necessary forms because of
insufficient coordination among programs. Neither staff nor parents
have accurate information to share with the child, and as a result,
