Transition Strategies: Continuity and Change in the Lives of Infants and Toddlers
 

Supporting the continued development of infants and toddlers with disabilities involves planning smooth transitions in services from Early Head Start to Head Start and from Part C services to preschool special education services. Developed for Head Start staff and managers, this report provides information about when transition begins, how to plan it, and how to involve families in the process.

Transitions occur at many levels, big and small, obvious or subtle. The obvious transitions such as entering or leaving a program or changing from one program option to another necessarily require planning and preparation. Other transitions such as developmental changes or moving from one activity to another in the course of a day also require thoughtful attention, but they can be overlooked because we may not immediately identify them as transitions.
